United Clinical Laboratories will take responsibility for transporting any lab specimens to Dubuque after the regular scheduled Mercy courier hours.
1. UCL-Dyersville Staff will package specimens and placed them in a small transport cooler for shipment, labeled with the place of destination. UCL-Dyersville staff will alert the receiving locations so they will be expecting the delivery.
2. Test not routinely done at UCL-Dyersville; staff may choose any of the options below:
A. If the patient is being sent to Dubuque via the ambulance, specimens may be sent with the ambulance crew for transport. Alert ER that the cooler is coming and needs to go to the lab.
B. If patient (or patient’s family) is transporting him/herself to Dubuque, the cooler may be sent with them. Instruct them to give the cooler to hospital personnel to ensure routing to the laborabory.
C. UCL-DV staff should contact another tech to either transport the specimen or have them cover “call” while they transport the specimen. Refer to the UCL-Dyersville call list. UCL staff will be reimbursed according to the “Standby Call and Call Back Pay” Personnel Policy plus mileage. Document on the “Payroll Additions” form and fill out the mileage form.
D. Contact a contracted courier to do the specimen transport. Refer to the list of contracted couriers posted in the lab.
a. Contracted couriers will sign an annual “Contract Courier Letter of Agreement” and provide a copy of their driver’s license and evidence of insurance on their vehicles(s). See Contract attached.
b. Contracted couriers will sign a "Confidentiality Agreement/Contracted-Authorized Healthcare Professionals" form. See form attached.
c. List of contracted couriers and phone numbers are posted in the laboratory.
d. The contracted courier will be reimbursed $75.00 per trip plus mileage. Use the “UCL Contract Courier Reimbursement Request” form attached.
3. Test routinely done at UCL-Dyersville – instrument down.
A. If the patient is being sent to Dubuque via the ambulance, specimens may be sent with the ambulance crew for transport.
B. UCL-DV staff will transport specimens during regular scheduled hours and another tech will be placed “on call” for transporting specimens to Dubuque as needed after routine laboratory hours.
C. Contracted couriers may be used during this time, but will not be placed “on call”.
