Note: All umbilical cord specimens are to be handled with "chain of custody" documentation intact.
1. Specimen Collection Kits:
Obtain additional specimen collection kits and "Chain of Custody" forms by calling United States Drug Testing Laboratories (USDTL) at 1-800-235-2367.
The Nursery and Neonatal Intensive Care Units have been supplied with specimen collection kits.
Note: Normal saline is required for rinsing the cord.
Each kit contains:
A. Umbilical cord container
B. Plastic biohazard bag
C. Box seal
D Cardboard box for shipping
2. Specimen Requirement:
The assay requires a 6-8 inch section of umbilical cord. Umbilical cord provides a sample that is always available for testing, easy to collect, offers an equivalent detection window and allows a higher level of sensitivity for specific drugs. Studies have found that cord sections were a more sensitive sample matrix for monitoring fetal exposure to methamphetamine, cocaine, and marijuana.
3. Specimen Collection (to be performed by nursing staff)
A. Place name and ID number on Requisition Form and the umbilical cord container.
B. Cut a 6-8 inch segment of the umbilical cord and drain and discard any cord blood.
C. Rinse the cord segment exterior with normal saline.
D. Place the umbilical cord segment into the umbilical cord container.
E. Complete the Date and Sealed by section on the bar-coded sticker from the Requisition Form. Peel off the sticker and place over the top of the matching umbilical cord container.
F. Select CordStat and 5 drug panel on the Requisition Form and complete the Collector Certification section. Make sure that the appropriate UCL/Hospital site return address is stamped on the form.
G. Place the top copy of the completed Requisition Form with tests ordered in the large pouch of the plastic specimen bag provided.
H. Place the umbilical cord container in the small pouch of the specimen and seal the bag.
I. Place the specimen bag in the cardboard box, close the box, affix the Box Seal at the indicated place, and initial the seal.
J. Place order for CordStat-5 in hospital information system.
K. Bring the sealed box and yellow and pink copies of the Requisition Form to the laboratory.
1. Accept the sealed box containing the specimen and the yellow and pink copies of the Requisition Form. (Do not sign the Chain-of-Custody. Only nursing staff are to sign and seal these specimens.)
2. Staple the yellow copy of the Requisition form to the UCL requisition and file in the Pending File.
3. File the pink copy of the Requisition Form in the laboratory legal consent file.
A. The sealed specimen collection box is placed in a Fed-Ex lab pak.
B. Specimens received Monday through Friday are picked up by Fed-Ex; the Fed-Ex driver stops routinely and checks for pick-ups on weekdays.
C. Specimens received on weekends (Saturday or Sunday) should be kept refrigerated and shipped on Monday.
D. Additional specimen collection supplies and pre-printed air bills are obtained by calling United States Drug Testing Laboratories at 1-800-235-2367 shipping supplies.
5. If there are any questions call USDTL at 1-800-235-2367.
6. Results will be returned by USDTL via fax within 24 hours after they receive the specimen.
When the faxed report is received:
A. Results will be entered and released in the UCL CLICS.
B. A copy of the faxed report is sent to the physician.
C. A copy of the faxed report is sent to the Nursery in an envelope labeled "Newborn Nursery Confidential Lab Results".
D. Finley Only: Send a copy of the USDTL faxed report for any positive result to Cindy Weidemann in Quality Management.
